WHEN APPLYING FOR A PASSPORT, YOU MUST HAVE:

Completed application but not signed

Certified birth certificate with seal (must be raised or multi-colored and not the hospital record). The birth certificate is mailed with the passport application and returned to you in a seperate mailing from your passport.

Two 2x2 passport photos (Alan Cross at Cross Flower Shop on Washington Street, Mark DeWitte at CompuPlus on Main Street, Walgreens, CVS Pharmacy, Walmart)

Driver License or government issued ID, preferably with photo

$75 check or money order made payable to the U.S. Department of State for adults and minors 16 or over

$60 check or money order made payable to the U.S. Department of State for minors under age 16

$25 cash or check made payable to the Hawkins County Clerk

Everyone applying for a passport must appear in person, including infants and minors. If a minor is under 16, both parents have to be present or the parent who is not present must complete a notarized consent form which can be picked up at our office or downloaded by clicking here.
It usually takes 4-6 weeks to receive your passport. If you need your passport sooner, you can expedite your application. Expedited applications usually take 2-3 weeks. To expedite, make your check payable to the U.S. Department of State for $149.96 for adult passport and $134.96 for a minor's passport as well as the $25 cash or check made payable to the Hawkins County Clerk. You will also have to make a check payable to the US Post Office for $18.30 to overnight your application. All fees are subject to change.
